Change of name deed
A Change of Name Deed is required if you wish to be known by an alternative name, for example, if you wish to revert to your maiden name.
The person completing the Deed must be over 18 and state their full name, as appearing on their Birth Certificate, and the name which they wish to be known as.
When the Deed has been signed by you and witnessed this should be kept with your Birth Certificate as you will need to produce this to confirm your change of name in the future.
You must retain the original document in a safe place. It is advisable to provide photocopies if you are required to send proof of your change of name.
